Health & Nursing

We are unable to accept any medication at school, including over-the-counter products, without the required documentation below.

Please review the attachments for these requirements and complete any required documentation with both the parent’s/legal guardian’s signature and the physician’s signature prior to bringing any medication to school.

  • Student Medication and Treatment Forms

    •  General Med Treatment Release Agreement – English

  • Allergies/Anaphylaxis

    The Anaphylaxis Medication Order and Plan is the preferred document for students with severe allergies who may need an EpiPen® at school. If the student requires other medications to treat allergies, such as Benadryl® (Diphenhydramine), please use the general medication form.

  • Asthma

    The asthma medication order and plan is the preferred document for students with asthma.

  • Seizures

    The Seizure Emergency Care Plan and Medication Orders for School and Childcare Settings is the preferred document for students who have seizures and may need emergency medications at school. If the student requires other medications, please use the general medication form.

Immunization Requirements

Colorado law requires all students attending Colorado schools and licensed child cares to be vaccinated against certain diseases unless a medical or non-medical exemption is filed.  For more information about vaccine requirements, resources and exemptions, please visit the Colorado Department of Public Health and Environment’s School Immunization page.

 


  • How to Get Vaccinations

    • Find locations that offer vaccines to prevent flu and other diseases on this page provided by the Colorado Department of Public Health & Environment. Check to see if a location offers low or no-cost services.
    • Denver Health’s School-based Health Centers (SBHC)  serve any DPS student at no charge to families. To schedule a vaccination appointment, call the Appointment Center at 303-436-4949.

Afterschool clubs are an excellent enrichment opportunity for our students, and we encourage everyone to take part in them! Keep checking this page as additional information will be coming as more clubs are added!

 

Boys and Girls Clubs

Kids in Denver can participate in after-school programs at DSST Elevate NE Boys & Girls Club. Membership is open to kids in grades 6-8 and includes STEM activities, sports, arts programs, leadership opportunities, and more. Click here for more information

An athletic fee is charged for students participating in middle school athletics. The following guidelines will be enforced:

· A fee will be charged for each sport at the middle school level,

· Fees are $30 per sport, with an individual maximum of $70.00 and $105.00 per family. 

· The athletic fee is due prior to sports uniform distribution and the first game of each season. Exclusion from participating in any event results upon failure to pay. 

· Fees may be reduced to $15 for all students who show evidence of qualifying for free and reduced meals.

· Athletic Fees may be paid at the school at the main office or Online. You can pay fees with Credit Card, Money Order, or Checks should be made payable to: DSST Elevate 

· Refunds will not be issued for:

          o Students who become academically ineligible,

          o Students removed for disciplinary reasons,

          o Students quitting after the first official practice.

· Refunds will be issued, on a pro-rated basis, for a student who sustains a season-ending injury, verified with medical documentation (doctor’s note).

· The collection of this fee does not guarantee participation/playing time.
